Intake Questionnaire

This short survey will help us figure out if our service is the right fit for you and your child. It should take about 10 minutes.

Once you’ve finished, our team of psychologists will review your answers. We’ll contact you within 1 to 2 weeks to let you know if our program is suitable and, if so, to book your first appointment. If it’s not the right fit, we’ll try to suggest other services that might be more helpful for your family.

Everything you share is private and only seen by our clinic team. The only exceptions are where there is a serious risk to someone’s safety, where there is reasonable suspicion that a child or elder is currently being harmed; where a court subpoenas this information, or if you give us permission.
